Please indicate the dates that you/consumer is expected to be absent, the reason for absence (illness, injury, vacation, Dr. appt., etc…), and a means by which we may contact you if we need further information or have questions. Absence due to Contagious or Possibly Contagious Illness

If a consumer is absent for an illness that is, was, or may possibly have been contagious he or she will need a note to return to program with specific information included on a Doctor’s Note. Please click on the illness form below for a sample of what to ask the Doctor to include in the note releasing the consumer back to program.

Absence due to Injury

If a consumer is absent because he or she has been injured to the point that he or she needed to seek medical care, he or she will need a note to return to program. Anytime a consumer goes to the emergency room for an injury, has a head wound, or if he or she has sought medical attention due to a injury he or she will need a note with specific information. Absence due to Psychiatric Concerns

If a consumer is absent due to psychiatric hospitalization and is ready to return to program, he or she will need a medical release to return. Please click on the psychiatric release form below for a sample of what to ask the Doctor to include in the note releasing the consumer back to program.
